If the student is a returning Al-Huda student, the office will need a re-enrollment Form. If the student is new to Al-Huda, enrollment is complete once the office receives the following:
Enrollment Packet or Re-enrollment Form filled out in its entirety. If the student is a new student we also need:
A copy of the student Birth Certificate. This certificate is used to verify the correct legal name and birth date.
Copy of Immunization records. All students entering any Arizona school are required by law to be immunized prior to the start of classes. Proof of immunizations (printed from your doctor’s office) or a signed waiver is required at the time of enrollment and must include the name of the person, the birth date, the type of vaccine administered, and the month, day and year of each immunization. Standardized Test results (if you have any).
Placement Tests: All placement tests only place students at an appropriate level. They are not used to determine grade level. Placement tests will be given during the first week of school.
